Carnitine's job is transport. Long-chain fatty acids cannot cross the inner mitochondrial membrane on their own; carnitine ferries them in, where they are burned for energy. Without it, fat sits outside the furnace.
Propionyl-L-Carnitine is carnitine with a propionyl group attached — a small three-carbon ester that changes the molecule's behavior considerably. It is more lipophilic than plain L-carnitine, which improves its passage into cells, and it has a particular affinity for vascular endothelium and skeletal muscle. Once inside, the propionyl group is cleaved and enters the Krebs cycle directly as succinyl-CoA, an anaplerotic contribution that plain carnitine does not make. So the molecule does two things at once: it delivers carnitine, and it delivers a citric-acid-cycle intermediate.
It plays a role in normal fatty acid oxidation, normal energy production in muscle tissue, and the maintenance of healthy endothelial function.
Carnitine is not, strictly speaking, essential — the body synthesizes it from lysine and methionine. But that synthesis is modest, dietary intake comes almost entirely from red meat, and the esterified forms are not made in the body at all in appreciable quantity. They have to be supplied.
